1-4 months: Hep B #2 of 3
2 months: DTaP #1 of 5, Hib #1 of 4, IPV #1 of 4, PCV #1 of 4.
4 months: DTaP #2 of 5, Hib #2 of 4, IPV #2 of 4, PCV #2 of 4.
6 months: DTaP #3 of 5, Hib #3 of 4, PCV #3 of 4.
6-18 months: Hep B #3 of 3, IPV #3 of 4.
6-23 months: Influenza, 1 dose every year.
12-15 months: Hib #4 of 4, PCV #4 of 4, MMR #1 of 2.
12-18 months: Varicella
15-18 months: DTaP #4 of 5
2-5 years: PPV #1 of 2 (high risk only).
5 years & older, through 18 years: PPV #2 of 2 (3-5 years after dose 1 if needed).
2-18 years (specific groups only): Hep A #1 of 2
2 1/2 years or older (specific groups only): Hep A #2 of 2 (follows 6 months afer dose 1, if needed).
4-6 years: DTaP #5 of 5, IPV #4 of 4, MMR #2 of 2.
11-12 years: TD Booster, 1 dose every 10 years.

Explanation of Vaccinations
Hep B (Hepatitis B vaccine): Protects against the hepatitis B virus, which is a chronic inflammation of the liver, it has life long complications.
DTaP (Diptheria and tetanus toxoids and acellular pertussis vaccine): This protects against Diptheria, Tetanus, and whooping cough.
Hib (Haemophilius influenzae type b conjugate vaccine): This protects against infections of the blood, brain, joints, or lungs (pneumonia).
IPV (Inactivated Poliovirus vaccine): This protects against Polio.
PCV (Pneumococcal Conjugate vaccine): This protects against infections of the blood, brain, joints, or inner ear.
Infuenza Vaccine: Protects against the Flu and it's complications.
MMR (Measles, Mumps, and Rubella Vaccine): This protects against the measles, mumps, and Rubella (german measles).
Varicella Vaccine: This protects against the chicken pox.
PPV (Pneumococcal polysacchoride vaccine): This protects against infections of the blood, brain, joints, or inner ears.
Hep A (Hepatitis A Vaccine): This protects against the Hepatitis A Virus, which is an inflammation of the liver.
Td Booster Vaccine: Protects against tetanus and diptheria.
